At one acre and above, the question stops being whether and becomes who may write it. That answer is set by your state, and it is the question the rest of this site exists to answer.

Where the requirement actually comes from

The plan is a condition of the permit that covers your discharge, not a standalone form. That is why the contents are federal (Part 7.2) while the preparer qualification is whatever your state permit says — and why a template downloaded from another state can be structurally fine and still miss the clause that matters.
